To obtain the Police Clearance Certificate from Ukraine the following documents should be submitted:

1. For Ukrainian citizens - copy of the passport - page 1 (containing the name of person and the passport number).

For foreign citizens - copy of the first page (with personal data) of international passport; copy of the passport page with Ukrainian visa and/or (if available) copy of the Temporary Residence Permit in Ukraine (or exact dates (period) of residence in Ukraine).

2. Completely filled out and signed application form.

3. Money Order/Bank Draft for 40 CAD (NOT a personal check) payable to the Embassy of Ukraine.

The documents could be mailed to 310 Somerset St. West, Ottawa, ON, K2P 0J9, Canada and prepaid express envelope (or envelope with stamps) should be added to receive the Certificate back.
