Completely filled out and signed application form.

Money Order/Bank Draft for 41 CAD, NOT a personal check, payable to "the Embassy of Ukraine in Canada".

The documents could be mailed to 310 Somerset St. West, Ottawa, ON, K2P 0J9, Canada, and a prepaid express envelope (or envelope with stamps) should be added to receive the Certificate back.
